IMPORTANT: Written notice of any claim must be be provided within forty-eight (48) hours of delivery or the

scheduled delivery date if delivery did not occur. This completed claim form and all supporting documentation

must be submitted no later than thirty (30) calendar days following the date of the incident. Submission of this

claim form does not constitute an admission of liability by Stallion Express, LLC. All claims are subject to

investigation and review pursuant to the Courier Services Agreement.
